Habilitation Vänersborg.

Here you will have the opportunity to work and be part of a multidisciplinary team where you contribute important pieces based on your professional expertise. The team includes, in addition to nurses, occupational therapists, physiotherapists, social workers, psychologists, speech therapists, dietitians, and doctors. We are now looking for a nurse for our clinic located at Vänerparken, near Vänersborg train station and the city center. We welcome your application!

Responsibilities

The nurse's responsibilities include assessing and evaluating health problems based on the patient's disability and planning and implementing interventions to give the patient increased opportunities to improve and/or maintain their health. All interventions are planned together with the patient and, if necessary, their network.

As a nurse, you will work with follow-up according to the national quality register for patients with spina bifida, follow-up of patients with multiple disabilities, and patients with neuromuscular diseases. Common tasks are related to questions about healthy lifestyles, pain, nutrition, breathing/circulation, skin/tissue, elimination, and pain. Consultation with the patient/network is a recurring intervention. Measures can be provided individually or in groups for limited periods. Digital visits are conducted when possible. The work primarily takes place in the clinic's premises and, as needed, in the patient's immediate environment.

Collaboration with colleagues in regional/municipal primary care and with colleagues in other medical specialties such as neurology, urology, and psychiatry is a natural part of your responsibilities. You often take the initiative for this collaboration. You often coordinate care contacts that the patient already has or needs. You are present in the profession at the clinic and have close cooperation with your colleagues in the team, where you work particularly closely with doctors and maintain close contact with our dietitian. You are part of a network with other nurses in habilitation, with monthly digital meetings and two meetings per year.

We are looking for someone who

• Is a licensed nurse with specialist training in open health and medical care
• Has experience, interest, and knowledge of working with individuals from our target group
• Is responsible and feels confident working both in a team and independently
• Is flexible, solution-focused, and has good collaboration skills
• Has a B driver's license as the job involves travel

It is an advantage if you have several years of professional experience, experience in regional/municipal primary care, and work in multidisciplinary teams. Personal suitability is highly valued.
